Key responsibilities
- Perform comprehensive patient assessments, including health histories and physical examinations.
- Diagnose and manage acute, chronic, and complex health conditions within their scope of practice.
- Order, interpret, and follow up on diagnostic tests such as labs and imaging.
- Prescribe medications and non-pharmacologic treatments, and adjust therapies based on patient response.
- Develop, implement, and evaluate individualized care plans in collaboration with the care team.
- Provide patient and family education on disease management, medications, and preventive health.
- Promote health and wellness through screenings, counseling, and risk-factor modification.
- Coordinate referrals to specialists and community resources, ensuring continuity of care.
- Maintain accurate, timely clinical documentation and adhere to regulatory and professional standards.
- Participate in quality improvement, practice management, and, when applicable, mentoring or precepting of other clinicians.
